a bit fuzzy. Tripod/monopod might have helped. My eyes wandered aimlesslyfor a bit. It might have improved if you waited for the cars driving by to have passed, (if passing cars ever did stop passing). The dismembered arm in the right hand side cost you a point,

I'll give you the benefit of the doubt that this is a 'city' hospital. If this is your child, C O N G R A T U L A T I O N S ! ! ! As a photo though, I have to say you cropped the top a bit too close to the finger and the horizon is sloping heavy to the left. Alos the hand(?) in the upper right corner distracted. (I know, I tell you you cropped to much from the top, then too little... No one said I had to be reasonable :)

Foreground is a bit too dark and it is overall a bit fuzzy. Was this a handheld shot with a lot of zoom? The more you zoom, the more you need a tripod. Also, I don't see the "Now" part.
