 With a home made light (from flashlight), expose the cig part first, then brush the light over the poison drawing.

Clone out dirt and hot pixels
Apply blurs, color balance, saturation/hue, contrast, level, channel mixer adjustments and unsharp mask

The moralism in this pic is likely to affect your score, but I bet you won't care. I like the execution of the shot. It's well-lit and the transition from smoke to skull looks failry natural. The big empty black space is functional as well. I have two minor issues with this pic: I think it is strange that the sigaret just hangs there and I think the pic would have been more convincing if the sigaret was burned only halfway.
