No. It is a Manfrotto sliding plate (357). Cost was $60 (Can$). The rest of the harware cost me $5. I had a groove machined onto the lower part of the L-bracket to make sure that the lens/cam was properly centered.
Also, my version only works if you have a tripod that has a video type head or three way head, ideally with a level. A ball head would require some way of rotating the head, like the OP.

it could be epoxied to the lower rear - behind the camera mounting bolt. or use a string level - but they only level in one plane.

Thanks for the level idea. I think if you mounted two of them, one on the back and one on the side, that would get you both planes. It could still be a little tricky with a ball head, but right now I am doing it using the camera and the grid lines in the view finder.
I can see this is going to be a work in progress with all of the great ideas people are coming up with. |

I added a couple of revised pictures to show the verticle mount I made vs. my original horizontal mount head.
