I like the reflection and the fact the bottle is out of focus in the background. the 'heart' blends into the pile of pills unless you really look for it. A separation would have improved this.

I think the image itself is beautiful! My problem would be with the border - because the outside edge is so dark (the lovely vignette) it doesn't work at all with a white border, to me, because my eye is brought to the border and the harsh line between black and white.. No border or a black one would be better, and that way the light part that stands out would be your subject alone.

What I like about the image is that there are only three "colors" - red, black and white. What I think could be improved is that the lighting is uneven. It is darker on the left-hand side. I don't like the bright reflection in the glass. If you had placed some black card on the right-hand side, that might have resulted in a black edge to the glass on the right as well as on the left and made the glass look more symmetrical. Lastly, the crop of the top of the glass seems a little tight.
