Funny, I had the initial same reaction to this, that she seemed to be struggling to push the bike up the hill but i actually thought the scene was flat. Only after reading through the other comments did I see that it was in fact a very steep hill! It is indeed a disorienting shot!
The orientation is nicely disorienting, and appropriately vertiginous. She really looks like she's working hard to push that bike along. Very Sisyphean, I agree with tnun.
