This looks pretty good except the tongue looks unnaturally pink. Maybe making it a little more subtle would have scored it higher for me :) Good tones on the B & W though.
I can see that you've made an effort here, and that is commendable (it means your trying to learn!)
That said, you have a nice conversion here with good contrast, and the puppy seems well-focused. However, its also obvious that you've used a blur tool to make everything besides the dog OOF. It can work well to bring attention to your subject, but ONLY if it is done very subtly.
What works better is to adjust your camera's aperture setting to a low number & use a shallow depth of field when taking the picture. Your results will appear much more natural, and will score considerably better.
