Nevermind, just answered my own question. The following was taken from a photo.net thread:
Canon made two adapters to enable use of an FD lens on an EOS camera. One is the FD EOS macro adapter which contains no glass. It's essentially an extension tube with a lever that allows you to step down the aperature. The down side is you evidently have almost no usable range of focus.
The other device is the one you referred to. It has glass elements and acts as a 1.26X teleconverter I believe. It was never available to the general public. Only accredited professionals were allowed to purchase it. The big problem with it is that because the way it is constructed it will only work on telephotos from 300mm and up. It will also work with the FD 200mm F4 macro if I am not mistaken. I have two lenses this adapter would work with but $500 is more than I am willing to pay for one. If I knew I was getting the real deal I might be able to talk myself into it. But I am very suspicious that an item that was produced in limited quantities by Canon is suddenly so widely available on Ebay. Maybe thats paranoia but I have seen counterfeit Rolex watches sold for $20 a pop in Hong Kong. It would not be that difficult to counterfeit this adapter.
