duplicate layer
spot heal blemishes
reduce noise, if needed
add an adjustment layer for levels
add the median
add a gussian blur and lower the opacity to 66%
erase the areas I want to be sharp
merge visible layers
resize image to 640 pixels for web
save

What do you guys think? Am I missing something?

What does it mean to 'add the median'?

A few obvious things missing...

1) Colour adjustment. Sometimes the WB needs a little tweak. Sometimes the colour saturation can do with a boost.

2) Lighten the eyes, and possibly teeth. I normally add a dodge layer, paint the eyes and the teeth totally white, and then reduce the dodge layer opacity to something reasonable. Often, it is less than 10% opacity, but it helps.

3) USM after resize. Given that you are using a soft-blur effect, you might want to USM on a seperate layer and only sharpen some areas, such as the eyes and other facial features.

4) Depending on the image, a subtle vignette can be a nice touch, just to focus the image.

Not every image will need all these things, but they are things that I often add.

RAW conversion - try to get colors and all settings right in conversion.

All this is as needed and in no particular order:
*Spot heal blemishes.
*"Pimp Eyes a la Cindi"
*Create new layer
Soften skin with a brush at about 10% ... sample skin color with each stroke.
Reduce opacity of this layer to taste.
* Or use Select Color range to select skin and then add a bit of Gaussian Blur, which I then fade to about 15-20%.
* Dodge/burn hair to add highlights
*Curves/Levels

If I'm going for web-sized:
I resize, smart sharpen, then add blur via Gaussian blur and fade.

I can honestly say that if I'm going to take the time to work on an image (ie touch) that the workflow is tailored to that particular image.
