I have sold a photo to a baseball card manufacturer and I need to find a sample 'limited use' agreement to cover this sale. Does anyone here have any examples I could take a look at?

I think alanfreed sold one of his Pittsburgh Pirates stadium pictures with a similar setup. Even if he didn't he may be able to help.
