A young gentleman was in my ER the other day with what is called a medial subtalar dislocation (ankle), and wanted pictures of it, to send to friends and relatives. He suffered this while riding his BMX bike on a half pipe. Rolled it inwards (medial). Said it made a friend puke, too! LOL
It is very impressive in the sense that when these do happen, the calcaneus bone (heel) does not go with the dislocation, and usually can be reduced in the ER. This one did and it went to surgery (closed-not cut open), where 3 pretty big men pulled on it before it went back in.
The last one I saw, we were able reduce in the ER, but when it went back in, some of the skin got stuck between the joint. Kind of humerous to us at the time. Not to worry, It was easily pulled back out.

Just the look at the foot alone made me sick...he needed foot care before he got hurt.

The little smiley at the end of my post was meant to imply that it was a toungue-in-check remark. I wasn't really implying that you should've submitted one of them. =]

I was aware. (C: I should have went for a close up on his big toe, though. YUM!! At least he didn't have Toxic Socks Syndrome!

i have to agree you should have kept them for the foot challenge....and OUCH OMG that had to hurt!

Here's my version, I did not take the picture. It was the hospital camera, a Canon digital elph.

so that one went laterally and no calcaneus with it? Ouch! How much pain meds? The young man before had 100mcg Fentanyl per EMS, 200mcg with us, 1mg Versed, and 1mg of Dilaudid before off to surgery. All in less that 40 minutes. He was about 105kg, too! (C:

Probably didn't expect THAT from DPC, huh?

That's not a lot of meds, specially considering Fentanyl is very short acting. When I worked in the GI clinic we used to give a minimum of 5mg of Versed and sometimes up to 100 of Demerol for colonoscopies amd MUCH MUCH more for ERCPs. I work in the ER now, such doses are not necessary....we don't wanna have to recover a pt for 1 hour :P

We gave her about 250 mcg of fentanyl iv in 50 mcg boluses and 5 mg of versed. I just pulled on it and splinted it. She also had a bimalleolar fracture with it and needed to go to the OR he next day.
