I've read david's tutorial on colours but am having trouble achieving this effect. Can anyone help me? I am using photoshop.

First off "Work on a Copy"

Go to Adjustment then
Hue/Saturation (CTRL+U)then
Edit:Master (Choose color than you want to turn b&W)
Saturation (Push the lever to the left: do each color individually)

without giving too much away, skin tones are causing me problems with this. Help!

is it possible to desaturate the entire image and replace the colour i want to keep? if so how?

It is possible by using selection tools, masking, etc...but it's not DPC legal.

Skin tones, if I remember correctly, are made up of reds and yellows. So, to remove skin tones, you slide the saturation bar to the left for red and yellow. However, to keep skin tones, you remove everything else. This is a very basic explanation - skin tones can be complex, and contain colors you may want to keep.
For example, say you want black and white everything but a single red item. This should be relatively easy - unless you have skin tones. One way to do this is to start off with the item you want a completely different color - green, for example. You desaturate everything but green, (removing the skin tones) then change the green hue (using the hue slider in the same toolbox) to a different color, in this case red. Again, this may or may not work.
