I use two Hue/Sat adjustment layers: the first to desaturate, the second to adjust the Hue for tones I like. Then sometimes a touch of curves, and some Selective Color.

Markus Hartel has an excellent color to B&W tutorial complete with downloadable files with curve adjustments and channel mixer settings.
Not to mention, his entire website and photography talent is freakin' awesome. |

I've been working with The Imaging Factorys Convert to Black and White Pro for the past year, and it is a great tool. Problem is, it aint cheap. There is a good review at luminous landscape somewhere, and I feel its worth cost. I had to spring for it after trying the demo.

| I use the Channel Mixer method, similar to the link to Marcus Hartel's site describes. In combination with a curves adjustment layer, it produces great tonality. I do wish for better support for duotones in Photoshop. It seems to make no sense to have convert to an 8-bit grayscale image to be able to do a duotone. Workarounds are time-consuing and clunky. |

i'm a fan of the channel mixer method, but i did just discover the exposure plugin and really really enjoy the effects you get from it.
this was done with exposure. i still need to get in there and bump up some of the highlights on the skin, but otherwise it's just a fantastic tool.
my 30 day trial is about to expire....what to do, what to do? |

I mostly use the channel mixer approach for conversions. I usually do it via Fred Miranda's "B/W Workflow Pro" plugin, which generates a nifty dialogue box that lets you adjust many, many parameters and view the result in real time.
I also do a fair number of conversions using the layered hue/sat approach, which works particularly well for not-quite-total conversions.
