So if he doesn't let go after five minutes or so should I assume that:

a) he fell asleep
b) the cancer is sneaky and hiding behind one nut, then the other nut...
c) it's time to call a lawyer

I think you've got it backwards, the classic symptoms of the "smallnuts" syndrome is a tendency to try to compensate for an overall lack of package size behaviorally. Typically this will behavior manifests itself as watching NASCAR, carrying a gun,and talking about how F&^$ed Up one got the night before and other such "manly" things.
