If it was a very fast moving train, it would be quite possible to get this handheld. Maybe a little longer exposure time showing the lights streaking further down the track.

If it was very windy, this could have been done handheld with a slow-ish exposure time.(1/2 sec?)

The excessively blown out street lamps and the faint ghosting of a couple walking on the right shows this wasn't done handheld.

A good telephoto lens with a high ISO, handheld? It's possible. Star trails with the moon would have been a dead giveaway.
