All just the lense

I noticed you used a Canon EF 85mm f/1.8 USM for this. I am guessing your aperture was set at f/1.8 perhaps? I am impressed at how you were able to keep the girl rather sharp and blur everything else with the exception of the floor just a tiny bit. The outline of her body (upper half specifically), is pleasantly sharp against the blurred background immediately thereafter. Some might question whether this was done in Photoshop since it appears so tightly done, but as you mentioned above it was all lense. I have an 85mm f/1.4 lense and understand the boke aspect, but how you got it so tight and close is really amazing to me. Great job! It makes me want to jump out and work on my depth of field control more.
