I wonder if a wider open aperture which limited the depth of field to the face might have helped to hide the creased background? |

My Inner Glowby mkothareComment: I know how hard it is to get these sorts of images to look good. If you have hollowed the apple out (which I think you have), I think some internal masking of the bright portion of the apple would've helped you to control the exposure a little more and get a more dramatic result. |
