The little shiny aluminum between the bellows and lens is the home made adapter. The lens can thread on it in either fwd or backward direction. The front element can be removed, just as you would take off a filter, and it becomes an awesome soft focus lens with about 60mm focal length. Soft focus is adjustable by opening or closing the aperture. The aperture in this lens has 19 leaves, so the bokeh stays pretty much round, even when stopped down. It's like a photographer's jewel, and it still looks great and works perfectly. It was less than $20 shipping included, on eBay.
It's a Bausch &Lomb Zeiss f4.5 for a medium format camera, mounted on a Vivitar bellows attachment. Pat date on the lens is 1903, and it was made before 1918 when the war started and ended the B&L- Zeiss connection.
