 Not sure what make this is. The lens is labeled as a "twindar lens"

Karen beat me to it! Odd that it's not labelled Kodak. How rigid is it extended like that? From here it looks like it would be unintentionally capable of some tilt.

The Twindar Lens is a Kodak lens, according to Camerapedia. And have a look at this - no picture to confirm, but the description sounds like your camera? Jiffy Six-20 - wait...here's another, and picture looks just like it:
