Used a flash on camera and pointed it up and to the left where I held a large white plastic cutting board.

Post process: I edited it a wee bit on Aperture: Crop, Saturation, Definition, Contrast and Levels.
Then I exported it to Photoshop: Unsharp Mask and added the small canvas.

Facts about suicide:

Worldwide there are more deaths due to suicide than to accidents, homicides
and war combined.

About 30,000 people in the United States die by suicide every year.

Alcoholism is a factor in about 30 percent of all suicide deaths.

I think stronger shadows or a more dim lightning could have made the picture stronger.

The sock lines are a little weird. I would have probably done some make up or something to make the skin look dead like it had been there a while. That would have given this a bit more punch.

Your photo made me gasp. Good work, that hasn't happened before.

I am torn between yours, and the other similar image, but as these are, so far, the only two that have moved me, both receive high scores from me.

All I'd say, is perhaps you should have waited a bit longer to take the shot so as to let the sock mark disappear on the right leg, but even so, the mark does kind of add some reality to it as there is no best or planned time to do the inevitable..!!

Think about the deatils, such as the sock mark on the right leg (when you're hanging, the blood will sink into your feet and they'll swell). What does the mirror add?
