 In a remote village in the Philippines. No time to explain it. Thought the grain helps.

This woman was elderly and sick in a village we visited in Philippines...Thus the title PAIN. She was asking us to pray for her, and we did.

Not crazy about your title. The expression has a certain ambiguity to it that you should maintain. Clearly, pain is part of it, but not all.
