This is a very poorly done photo for many reasons: 1, it isn't appealing. 2, it's neither here nor there, it's got no relevance. 3, there's no technique - composition, lighting, flattering angles, interesting assertions. 4, it's a stretch! "his last breath" = "broken" doesn't work. ultimately, it's just not an attractive photo, which is why i scored it really low. sorry.

This evokes a kind of emotion that is not relevant to the challenge nor to DPC in general. Your title implies his impending death and is disrespectful in my opinion.

Are you having an experiment seeing how many challenges you can enter with a portrait vaiation of your/models face? Just curious. If you are then I am really excited to see the results.

Not very flattering, which will probably lower the score. Besides the title, something more is needed to tie in that this really isn't meant to be flattering. How about something like a first-aid kit, oxygen mask, even a beer bottle ...

Generally with portraits the eyes are the most important thing to get in focus, and I'm afraid this is something that you did not manage.
