2nd submission. First removed because I had forgotten about basic editing! (It's been a while since I submitted.)

Softbox behind him. Large vertical reflectors angled at 45 degrees on his left and right, bouncing light into his face. Metered from the bounce.

Very striking. I'm wondering how many people are going to interpret this as creative use of shallow DOF.
I don't profess to have a lot of technical know-how but this seems more like creative use of high contrast and high key lighting. I guess to achieve this fading into whiteness effect you had to start by blurring the subject a bit? But couldn't that also be the result of really extreme highlighting? I really like the photo quite a bit. I just am not getting shallow DOF from it. The bangs look a bit blurred in the front but I feel like I'm grasping to make this meet the challenge. I'm guessing you are going to get plenty of high votes from people who don't mind shoehorning a photo to fit a challenge if they really like the shot. I'm not really sure how to vote on this.

Most of picture is in focus that is not shallow DOF

I don't really like how one side of the face has blown out highlights - it's a bit distracting because the ear just suddenly appeas in the white space.

Good composition, good expression on the model - overall a nice picture.
