When I see this type of photographs, often I wonder what the next photograph would be. In other words, what's the reaction of the person knowing or seeing it's getting his photograph taken.
The dissonance between the facial expression and the soft pinky message works to deliver a strong message. What is it? Is that a doctor behind the closed door? Marvelous grain, btw.
