Some one I know from another forum took this image. The spots aren't the issue, it's the streak he is confused about, what is it, and what causes it?

Image is quite large for posting.

Looks like light reflecting off the lens.

To me it looks like the light somehow reflected off her ear ring and hit the lens at just the right angle to cause a flare.

A ray of light from the earring hitting a strand of the photog's hair, or a spider web strand across the lens could cause that if it is only in the one shot. That's how a star filter works. It's just a fine screen placed in front of the lens, and it makes a star at the bright points in the image. Example:
