Is there a certain form? If not, does anyone have one they'd be willing to share? I suppose I do need to start gathering them =)

Here is all the wording I have on mine (this one is set up to do over the web, but I have the paper release that I fill out to send/give the model.
I read about 5-6 different ones and came out with these.
The permission of use is the "non-scary" wording.

Here's a link to mine in PDF format.

I print these in duplicate carbonless at Kinko's and have them gummed together (glued at the top). I keep the top copy while the model gets the bottom (yellow) copy.

The next revision will have a line for what exactly they are getting as compensation. Currently I write their compensation (payment or photos & CD's) in the blank space.

Just so you know, it is quite hard to read them on your background.

Yes, you're right TerryGee but if you click on Edit|Select All, this will make pages like this easier to read.
