I was wondering if any of you would post an example of the form/paperwork/whatever you use for making your photo appointments / bookings. What I am looking for specifically, is a good example which allows you to put the customers name and personal information, whether they have paid a deposit, paid the entire sitting fee, what type of photo shoot it is, etc... I have excel if any of you want to send me a sample in .xml format.
