Does anyone have a image/release form that I can have? I googled it and found one that worked some what but I don't know if it was everything that I am looking for.
I need one that allows someone to buy an image that they can use for personal reasons and then one that I can give to clients that buy CD's from me.

You sound like you want a licensing agreement/contract rather than a release form, which are typically permission to use/sell the likeness of a person or property ....
You might see if Nolo Press has any, or copy one from a stock supplier.
