I did a shoot out in the rain last night with my roomate, playing with my new 430EX, lightsphere, and Cactus V2 transmitters.
On a few shots, I had him hold the flash. They turned out pretty cool, but the flash seems to be showing up twice in the shot, a second time seemingly rotated 180 degrees around the center point. I'm guessing it has to do with light bouncing around inside the lens.

Is there any way this can be avoided?

Looks like you were having fun. :-) I have no idea on the second ghost image, but will watch this thread to see what the answer is. |

Were you using a UV filter? |

UV lens is always on the lens as protection, and seems to be the culprit
I'll be sure to take it off next time i do shots like this.
