I've been asked to have a try at removing the bottle, glass and arm on the right-hand side in this picture. I've tried a few times but because of the guys waste coat (you might need sunglasses!) I'm finding it difficult.

Can anyone help me out?

P.S. It's actually a scan of a normal photograph, not an original digital image.

Yep, needed the shades...
The pattern on the vest doesn't seem to repeat nicely, so there's no easy way to copy it across. I would have suggested masking off the area to the right of where the vest would end (but wxactly WHERE??) and then copy/pasting and adjusting the pattern to match, but since there is no visible pattern repeat, I think this will yield sub-average results.
You may try just burning to reduce the brightness of the bottle, also some distractions along the left edge, etc.
Some of the other nuiscances may be easier to deal with, but you're looking at a significant time investment.
There's also a great deal of noise in the shot, don't know if this is in the original or from the scan. If the scan is visibly adding noise, try scanning several times, then stacking to reduce the noise.

The photograph is quite an old one and the quality isn't brilliant. Scanned it in using different resolutions, this is the best I could come up with.

My advice is to just crop above the bottle and make it a horizontal. If you need vertical, crop as tight to his head as you can, removing as much of the bottle as possible, then darken the bottle in PS.
