I've noticed that a couple of times in the past photos have been disqualified after they have won a ribbon for something that is very minor and hardly noticeable. How are these noticed? Are all the winning photo's checked for spot-editing and so on?

Sometimes it is obvious. Sometimes someone has seen the photo elsewhere on another site months before the challenge, sometimes the photographer admits to it, not knowing the rules. And sometimes we check, just to be nosy. :)
