I used to have an older tripod (not manfrotto) that had a reversible center column with a cap. We removed the cap for ease of switching column positions, and one time my assistant neglected to set the screw in the collar before releasing the column after adjusting height. The whole thing dropped like a stone and smashed a perfectly good Nikon.
I've had plenty of occasions in the past where I've let a column go when adjusting it and had it slam down to the collar; fortunately, always in the upright position, not the reversed position. So I'd say "yes, this is a very real possibility"... Just remember Murphy's Law; do you really want to test it on your camera gear?

Yeah, I wish Bogen would implement a revision using a connector. So you could upgrade to the type of mount.
I mean, the ball head is almost identical except for the mount. Bogen could make it so the mount just locks in. This would be nice in that you could upgrade to various plates (be they RC2, RC4, the hex or any future design) without losing your full investment into various heads.

Fortunately it wasn't a Canon...LOL. Sorry, I couldn't help it.

Before you make the jump to a proprietary system like Bogen's I'd recommend looking into the universal arca swiss style system. Besides being by far the strongest way to hold your gear it is also the most customizable. Once you use a L-plate, you will never flip your tripod head over again. In the same price range of the bogen head, you could get a FLM or Giottos head and convert it to Arca Swiss. Yes, the negative is that the plates & clamps are slightly more expensive, but will last forever.
Also many more customizable accessories are made and can be made with aluminum bar stock from hardware stores to fit the open channel AS system.

Just to illustrate my previous point I looked into my photographic junk drawer and found some of the numberous quick releases I've used before switching to the arca swiss system.
Note the duct tape on the bogen release. It can get loose over time.

I've got the 3221WN with the 488rc4 head on it and the 2nd best thing I ever did with this tripod was swap out the standard long column for the shorty version. This lets me get closer to the ground. The best thing I ever did was swap out the shorty version for the leveling 555B column. This is great for getting my head level for panoramas. First I do as close a level of the legs as I can. Then I lossen the column head and level it. Then I level my ball head. Now I can do a 360 degree spin and the head stays level. If you do any work where you want your tripod level over a good portion or all of the arc of your head, this is the column to have.
