I've pulled a number of different model release forms off the net and used bits of various ones to create my own.

I'm looking for something nice and short so that it doesn't intimidate people into not signing it.

How's this:

Photographer: Kavita Favelle

I hereby give permission for unrestricted use of any photographs of me taken by the above-mentioned photographer. These photographs may be used for purposes including but not limited to art, advertising, commercial, editorial, educational, illustratory, promotional and web. I accept that full copyright for the photographs taken resides with the above-mentioned photographer. I agree that I am not entitled to any further payments in respect of the use of these photographs.

A contract needs to give both sides something to be legal. All the releases i have seen say 'for good and valuable consideration...' (usually a print although this is never specified). the rest seems good. I borrowed mine from alamy mostly i think so it long and has weird language in it LOL.

I'd lose the payment line at the bottom...

It sums up what you want to say.
This is mine and I think that there is a lot to fill out but it is useful information so i can contact the customer ifn the future for any reason.

I usually have a witness just in case there are any problems.

Model Release Form

I _____________________________ (The Model) explicitly grant to the photographer Ben Rawson (The Photographer) and to his assignees and licensees the absolute right and permission to use, publish or sell the photograph(s) referred to below, in which I am included, in any medium, throughout the world, without restriction whatsoever as to the nature of the use or publication or as to the copy of any printed matter accompanying the photograph(s). I understand that the images may be altered and I waive the right to approve of any finished products.

I understand that I do not own the copyright of the photograph(s).

I agree that I am not entitled to any payments in respect of the use of these photograph(s).

I certify that I am over 18 years of age and that I have the full legal right to execute this agreement.

Maybe substitute "above-named" for "above-mentioned"? And "grant" for "give"?

Or:

"I hereby grant Kavita Favelle ("Photographer) the unrestricted right to use any images of me taken on this/these date(s) for any purpose(s) whatsoever, including (but not limited to)..... I understand that the full copyright for these images resides in the photographer. I understand that I am not entitled to any further compensation for the use of these photographs."

Yes, it looks good to me, too.

Two small suggestions. The wording "any photographs of me taken by the above-mentioned photographer" seems to give you permission for all past and future pictures you take of the person. But you probably mean just the pictures taken on the date and location filled out below in the form.

I often pay subjects with prints of the photo. They, of course, don't get the prints until later. Instead of "Payments received," I would say just "Payment" so I can fill in something like "two prints" without the subject worrying about signing something that says they received the payment.

Thanks for these comments, folks... very useful.

I should have clarified further - this particular model release is for my upcoming trip to India and I definitely don't want to offer to send any prints or the like. It's either going to be a case of no paymeny or a small payment made at the time.

I suspect I will probably not be able to get too many to sign the agreement at all as I'll be shooting a lot of candids but I may be able to negotiate to take a series of planned/ posed images of certain individuals in which case it might be useful to have this with me.

I wanted to make a note somewhere on it of any payments made but you think it's best to leave this out completely?

Given there are SOOOOO many languages in India that would be pointless as which Indian language would I choose? But since English is one of India's official languages and spoken by a majority it should be OK. Besides which I don't speak any of the Indian languages so couldn't converse with anyone unless they speak English anyway!

I believe Hindi is one of the most common languages, so it might be worth having somebody look into that in addition to English if it's a serious use.

Hindi is indeed a common language, especially in Northern India where I will be. I should mention that I am Indian, though born in the UK, and have been to India many many times. It's to my shame that I don't speak hindi myself.

I don't think it will be necessary to have the form in two languages because English is the official language in India, especially for official stuff, and I will only be dealing with people who I can speak to, negotiate with and introduce the form to in English.

Oh - and you may find it useful to take a photo of the person holding the signed release near their face - good proof of authenticity (in case you needed it) and easy to identify names to faces etc when you get back!

That's a bloody marvellous idea! And if I can't get them to do that I can take a picture myself of the form so it appears after the photos of that person!
