when I need to do non-standard size, like panorama, I use a service called FotoWire. You have to download a small client to use it (from www.fotowire.com) but then you get to choose from a huge array of photo labs (most of them mom & pop shops) that offer every conceivable size and finish between them. The widest-aspect panorama option I've come across is 4x12 and it was something like 75 cents. Some of the labs have high prices but most of the labs are quite competitive, especially considering the non-standard sizes. Cropping is done with the client and is very quick and easy.
