Can someone email me an image release form allowing clients to print images from a CD???
I took some family photos and just gave them a DVD so they could print their own, but I would like to add a release form to the dvd.
Do you have one I can look at? |

Joe,
I don't have an image release form, but do you have a model release form that you use? If you do, you can just add a section that says the client is allowed to print, duplicate, and/or reproduce photos for personal use. |

Here is what I have to use....got it from a DPCer a while ago.
Customer Name and Address:
Date of Sitting:
Subjects: (Full names of all people in photos if available)
Description :
I, _____ , grant unlimited printing release to:
This release is fully transferable by the above named FOR PRINTING ONLY,
and a copy of this document should be considered adequate proof of release by the printer.
This release does not include rights of sale, nor is permission granted for contest entry
without written permission from _______.
Reproduction rights of described photography transfered only if signed addendum to this form has been provided
to ______ by the above named customer
Copyright is retained by _______ for the above described photos.
Note for printer: If further information is needed per your company policy or verbal confirmation,
please do not hesitate to call the above number. THANK YOU!
