I'm trying to get the eye to see things in a certain order:
1. Orthotic, Foot, & Shoe
2. eye travels up leg to Logo
3. Handcrafted Custom Foot Orthotic -and- Quality Footwear
4. The title and website as a followup to the interest in 1-2-3.

My eye goes to the logo first. Sharpest and brightest object in the image. I don't know of a company who would consider this a bad thing though.
Then I read Quality Footwear, then the verbiage to the left. I then want to see what the third piece of writing is about, so up my eyes go to that. I do think companies probably want their name and site info equal to or larger than the other writing (it's about buying the product FROM THEM not just about buying the product).
Finally I see the blue line (the ortho shim) then my eyes move through the shoe until the toe is identified and the rest of the pale tan area makes sense (rest of the foot behind the toe) and the entire shoe area sort of comes into focus all at once.

Your best feedback will come from the company, of course. You may want the company name in text as well as on the tattoo (nice job on that, by the way!), probably as the top line, and probably a bit bigger than the other text you have there. The first thing I "read" is "Quality Footwear", then the part about the orthotic.
Really coming together nicely! |

I've a couple of things that bother me with them.
Main pic:
1) The skin tones on the leg look blotchy which is a bit distracting
2) I would use a crisp / clean cutout to show the foot / sole separation, the blurred version has a tendency to make the shoe look tatty, it also takes a while to work out what is going on, but if you have a cutout with a solid line breaking the foot / sole and shoe up I think you will get a much crisper look.
Second pic:
I would have the shoe solid in this one, no need to have the foot bleed through at all.
Overall though nice images with a nice clear message.
