In Photoshop: when printing with a custom printer profile, should I convert color space to CYMK or RGB?

Not unless the lab tells you to convert to CMYK. The lab usually wants to do the CMYK conversion themselves.

If you're printing yourself then you don't need to the convert from the workspace profile to the printer profile. Photoshop or the printer driver will do that for you.
