Yep, needed the shades...
The pattern on the vest doesn't seem to repeat nicely, so there's no easy way to copy it across. I would have suggested masking off the area to the right of where the vest would end (but wxactly WHERE??) and then copy/pasting and adjusting the pattern to match, but since there is no visible pattern repeat, I think this will yield sub-average results.
You may try just burning to reduce the brightness of the bottle, also some distractions along the left edge, etc.
Some of the other nuiscances may be easier to deal with, but you're looking at a significant time investment.
There's also a great deal of noise in the shot, don't know if this is in the original or from the scan. If the scan is visibly adding noise, try scanning several times, then stacking to reduce the noise.
