A simple google search would turn up plent of results, but here's one that I use.
It's a combination of many that I've found AND it's in plain english as much as possible - less confusing for the models (AND the photographer ;P )
I've bold italicized the gender-specific references, etc that you tailor to your own. They don't appear that way in the printed version.
MODEL RELEASE
In consideration of my engagement as a model, upon the terms herewith stated, I hereby give to yourname and yourcompanynamehere (collectively, “the Photographer”) his/her heirs, legal representatives and assigns, those for whom the Photographer is acting, and those acting with his/her authority and permission:
a) The photographer is authorized to use my photographic images (and derivative work) in both edited and unedited form for both private and commercial purposes without restrictions in time, context, location and content, this to include any accompanying text. This includes publication on the Internet, display in exhibitions, the distribution of prints and publication in print media, as well as the right to grant or sell these rights to a third party. I (the model) will receive a specimen copy in the event that my image is published in print media.
b) I, the model, am authorized to use any photos produced by the aforesaid photographer in both edited and unedited form for private noncommercial purposes without restrictions in time, location and content. Furthermore, I (the model) am authorized to publish or display my photographs for noncommercial purposes in unedited manner in physical print form or in digital form in any media for self-advertising purposes (zed card, portfolio, Internet page, etc.). I (the model) am obligated to give credit to the photographer in these instances. I (the model) understand that I am prohibited from selling or distributing prints commercially and may not grant copyright to a third party.
c) I hereby release, discharge and agree to save harmless the Photographer, his/her heirs, legal representatives or assigns, and all persons functioning under his/her permission or authority, or those for whom he/she is functioning, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.
d) I hereby affirm that I am over the age of majority and have the right to contract in my own name. I have read the above authorization, release and agreement, prior to its execution; I fully understand the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.
