I have been asked to shoot a small prom for a local high school. They are paying me a flat rate to shoot the event and want me to put together three different print packages. Any suggestions or guidelines on pricing/markup would be appreciated.

ask them what they paid last year. if they don't remember, find someone who does remember. if it's a fair and reasonable amount, take it. if not, ask for more. if they don't like it, then walk (or suck it up and take it).
it's really best if you can get your hands on one of last year's packages; otherwise, you might be told something that is rediculously low... |

Around here mostly it's 2 5x7s in carboard folders (frames that stand up). Your cost varies, but $2 is about average. Sales prices can be $12 to 16 depending on your market and what last year was.
They're paying you? Hot damn! It's more normal you pay them!
As for other packages...2 4x6s and 2 8x10? Wallets may be popular, and these days magnets are hot.

Well, they are looking for packages that are much different from what they have had in the past.
Parent Package
2 - 8x10
4 - 4x6
8 - wallets
Couple
2 - 8x10
2 - 4x6
4 - wallets
Friend
1 - 8x10
2 - 4x6
Since they are paying me a flat fee to shoot ($200) I don't want to price the packages so high that no one is interested but I want to make some money on them too.
I guess what I am trying to do is arrive at a fair price.
Any other suggestions or comments?

Depends on where you are...here an 8x10 at an event is $12-14 and wallets command a premium above that (even though it's still 1 unit).
My one lab can do the Parent Package for $3.25 with cut wallets. Another lab wants nearly $15 for that ($3.17/unit for their best work) or $5.48 for their econo line ($1.87/unit), and yet another has an 'event' line of products..$1.20 per unit you CC, $1.56 they CC, and they'll bag each order for $.60 including envelope.
So does pricing mean much in relation to the cost? I know pro labs that choose each level of product...if you want to make $10 then you could charge $13 or $25...if they parents will pay $25, then use the cheapest lab and make $22!
What did they sell in the past, at what price? What does the little league photographer sell pics for? The school photographer? THis will give you some idea of what they parents are used to paying.
Me? $35 for the parents package, and i doubt you'll sell any.
The couples...usually it's 5x7s...kids don't want 8x10s of themselves, so I'd add some package with 2 5x7s and 8 wallets for $20.
Friend? Again, 5x7 and 4 wallets for $10 to 12.
If they insist on the 8x10 packages, then offer them ALSO - and price them a bit over the 5x7 deal. $25 and $15 maybe?
Here we have to charge sales tax on something like this, 6%. It's not worth the hassle of trying to collect pennies and change, do it's factored into the final price....so a $10 package is really more like $9.42, and $2 to $3 in cost (plus labor to sort and deliver the order) , so you get $5.42 to 7.42 out of that $10.

My son told me of a photo op they had at his semi-formal that sounded good. Group shots at $5.00 each for a 5x7. His picture had at least 20 kids crammed into the one spot. 20 kids times $5 is $100. 5x7" from MPix are $0.89 each (quantity 10+) so that's a cost of $17.80 plus $4.00 shipping. So you clear $80 a group. They are having fun and the parents (like me)gave them money and he has a decent picture of a fun time.
