I'd guess his technique either uses a high-grade diffuser filter or an out-of-focus enlarger. It doesn't look like the camera's lens was out of focus because then the far away objects would be more blurred than the close ones.
I've use the vaseline trick, but leaving the center of the filter clear so only parts of the image are diffused:

ok, well I admit I haven't read all the responses here, so if this is double on the info I'm sorry.
Ken Rosenthal is one of my most favorit photographers. I know there is a group of people trying to do this digitaly but thats not it at all. he uses a 4 by 5 film camera, sometimes useing a pinhole lens on the camera. I have a very good friend that is doing it rather well with photoshop, here is his deviant art account, he really is something else
now to call jeremy and hoping he doesn't get mad at me for showing him off....
