Does anyone have a copy of a simple "consent to use photo" form that they can post or link?

try modifying the form found here:

or here:

//www.bio-link.org/pdf/photo.pdf

There are certainly A LOT more complicated ones out there, but these ones, with some tweaking might just do the trick (if I understand correctly what you're looking for)
