I currently have a Model Release Form that I use...but it is quite a pathetic layout to say the least.
Can anyone recommend a good one please. I need them pretty urgently and will need to print out about 400 of them for a job in a couple of weeks.

Sweet - 400 model release forms? That's about 392 more than I have on file.

This is the one I use, simple and to the point.
Model Release
DATE _____/_____/_______
For valuable consideration, I hereby irrevocably consent to the use and reproduction by Roy Hill or anyone authorized by him, of any and all photographs that he has taken of me, negative or positive, for any purpose whatsoever, without any further compensation to me. All negatives and positives, together with the prints, shall constitute your property solely and completely.

Signature________________________________ Date____/____/_______
If model is under 18 years of age. Please provide a parent or Guardian signature.
