I was shooting a comedy show on Monday. Noticed that many of the shots were very soft even out of focus. Certainly a higher percentage than I am comfortable with. Anyone seen this before?

Could be a number of things. Shutter speed, ISO, using a lens wide open, narrow DOF, missfocusing, subject/camera moving slightly after focus lock... all examples, we'd need more information to make a good judgement of what it is.
