There's a lot of talk in these forums about model releases, but I don't recall anything about music. Granted, this isn't strictly a photography question, but it's related if you make slideshow CDs/DVDs for clients.

I am planning to contact a couple of local bands & individual musicians about using some of their music on for-sale slideshow discs, but I have no idea; what to offer them in terms of money, where to find the proper release/license forms, etc. Anybody have experience in this area?

Take a look at Peer Music. The actually do licensing, though I've never needed, just know that they do it, one of my clients.

-edit: this might give you an easier idea about cost to license music, the band may have no idea. But hey, neither do I.
