'set up' in my opinion is something scripted, planned, arranged.
For example, you go for a walk in the woods with your dog. you see a bridge. you have Rover run across the bridge while you take shots, 1 or 100, with the thought of 'making' a photograph. VS you are our to take pics of flowers in the woods, Rover is running about chasing a butterfly, you take a pic of him and the insect. No planning, no set up. You may still take 1 or 100 shots.

Control lighting as in create it? (sunset vs strobe) or as in using a reflector or fill flash? I can take Rover out and without altering the Sun;'s rays, take 6 different shots regarding light. I may not control that light source, but I control how it interects with my subject and the camera (bright sun, shade, dappled shade, silhouette, etc).

I far prefer spontaneous unplanned shots, or at least those that look that way. The studio stuff doesn't do much for me really.

All my highest rated shots here are set-up indoor stuff though. I think if challenges were always free studies, that wouldn't be the case. Getting something topic-related when I don't have more than a few hours a week to do photography makes it hard to submit much else. It's either participate with set-up shots or not at all, at least most weeks.

as with ed's example, i think the distinctions go a bit beyond 'natural' and 'studio'. i tend to describe them as spontaneous moments (12), found locations (8), staged (8), or studio (7).

of these, my staged/studio shots are really hit or miss, markedly above or below average. my spontaneous moments seem to do average or better, and my found locations are about even.

all the same, by pushing myself to try to learn a lot of these things that i never would have tried before, it has really opened up what i have been able to do with my personal photography.

Well.. i love taking Natural shots. I only submit 1 shot in the old and new challenge, and now is framed in a my house wall.

Im just playing with "studio" shots (my office table, with some lights) so im learning, i think i will start doing the studio thingy after i learn something more.
