I've noticed at times when I'm in full manual mode including manual focus, I press the shutter button and it doesn't fire... Has anyone else had this problem?

It's quite possible that you have the option selected that prevents the shutter from opening unless the autofocus is locked onto something. This option is selectable in the programming section of the menu.
The autofocus lock is indicated in the viewfinder with a circular dot that flashes while it's focusing and goes solid when it locks. |

Could this happen while in manual focus mode too if the subject is not in focus? |

Yes, I believe so. I don't have the camera with me right now, and I'd have to experiment to confirm, but I'm fairly sure it can/will happen.
Edit: misread you post; I believe it will happen in full manual mode, however I don't think it should happen if the lens is set to manual focus.

Not sure about the 10D, but on the 20 I had some similar problems when I first got the camera... I was experimenting with the different options in the custom functions..
From memory it was the AF/AEL settings that caused the problem, even in manual focus.
It didn't seem to be consistent, but resetting all settings to default fixed it, and I've not seen the problem since.
