Interesting photo. My frist thought was..."What terrible thing did he do?" But, after looking at it for a bit, he could have done something good. Finsihed operating and such. I like how it doesn't give the answer. It makes you think and find what you want too find within it.

Like the choice of lighting here, makes it look as though it's in an inclosed area that's dark. The only nit pick would be the lighting on his left arm. 8

In my opinion this would have worked a lot better if the focus was everywhere, and not predominantly on the hair. Also the two fingers are not necessary, and are distracting.
