Wow, I came to this image because of the "least favorite" thread. I thought that I was going to hate this because stuff coming out of mouths like this is not my favorite! But I like it!! The editing is wonderful. I just love the awesome shade of blue in the background :-D

Curves adjustment masked to paint in highlights.
Curves adjustment masked to paint in lowlights.
Tons of dodging and burning.
Selective color to bump up foam, brush and bg.
Smart sharpen.
Highpass filter set to hardlight.
desaturated layer set to overlay.
resize for web
smart sharpen again.

Its all painting really..Layer masks are the way to go.
