In an effort to increase the DOF and keep her arm sharp (there's no reason to have it slightly soft like it is), I would have changed settings to f/8 (lose a stop), ISO 400 (gain 2 stops) and shutter 1/8th (lose a stop). Then you could use Neat Image to remove any noise that resulted from ISO400 (although it is quite manageable at this setting). 1/4th is also the perfect speed where vibration from the mirror can cause problems. If the 350XT has lockup, I would have used that. I don't think that's the reason why her arm is soft, but it's just a tip.

The pose is nice. I like the composition and I like your conversion to B&W. The large earring is perhaps not my choice as it flares quite a bit there; but that's not a fatal flaw. I bet some of the low score came from people merely not thinking a model qualifies as stationary enough. The highest picture finish primarily using a model that DIDN'T contrast it with something in motion was 32nd...

I don't see why this scored as low as it did, unless people thought it DNMC. It's potential a great portrait, hurt only by some lighting problems -- the dark shadow on the far side of her nose, and her right shoulder both detract from a well posed, framed, and exposed picture.

I feel really disappointed for you that this finished so low...this was an excellent shot. 4.991 is not a true indication, and maybe in another challenge it would have done better...sorry that the voters didn't give this a better score...I LOVE it!
