It wouldn't be obvious what this is without the title and the tyre marks in the lower part of the shot are a bit distracting.

Good idea. The fingertip needs to be a tad sharper though. Maybe include just a little more of the face and a little less of the hand at right. The red toys (??) at top are a distraction, and could be removed prior to shooting for a cleaner composition. A cleaner compo of this might also look very good in black and white. I think I would also try to remove the piece of blanket between the hand and the face, or put it further back, so it's softer. ~6
